We know that these are some big changes and are honored to be entrusted by the Physician’s Building Group team to continue to carry on their mission and values in serving you and your family. Similar to Physician’s Building Group, we are an organization that is Oregon owned and operated, putting patients at the forefront and striving to preserve the neighborhood clinic feel. We recognize that providers, patients, and staff have a choice in where they work and who they trust with their care. We are excited to partner with Physician’s Building Group as the team looks to transition to the next chapter of serving patients with Praxis Health. Inevitably there will be some changes and challenges as we make this transition, so we ask for your patience and feedback along the way.

Starting October 1^st, Physicians Building Group will become a member of Praxis Health. A lot will stay the same, however, there will be a number of changes as you come in for care this fall.

• Your Provider Team will continue to provide care at their current location through March 2022.
• We are remodeling our South Salem location to serve everyone in one space.
